DYNAMICS IMMUNOLOGICAL INDICES IN PATIENTS OF CHRONIC GENERALIZED PERIODONTITIS DURING THERAPY WITH MELLINOL

With the purpose of correction of immunological changes in dental patients has been studied the influence of the drug mellinol in the treatment of generalized periodontitis in 32 patients with chronic viral hepatitis b and C. The results obtained show that in the group of patients with chronic viral hepatitis b and C using the drug significantly increased rates of T-lymphocyte subpopula¬tions of T-helpers, a marked increase the number of B-lymphocytes and natural killer cells, and increased the number of cells expressing a chain of the IL-2 receptor and receptor to transferrin that shows a marked improvement periodontal status among the surveyed population. Initially reduced rates of T-lymphocytes and T-helpers, expression markers of activation, as a manifestation of immune dysfunction in patients with chronic viral hepatitis b and C, during the therapy with mellinol significantly increased. When using the drug in the least degree of expression is subjected to a subpopulation of cytotoxic T-lymphocytes, which are responsible for the clearance of the virus and the production of cytokines, controlling viral replication. It is revealed that the immunotropic effects of the drug mellinol of periodontal patients with chronic viral hepatitis b and C is manifested positively, regardless of the type of virus that caused the disease, and correlates with the notable restoration of fabrics of parodont. -

Diagnosis and surgical treatment of occlusive lesions of brachiocephalic arteries

Observed the results of observation and surgical treatment of 102 patients with occlusions defeat of brachiocephalicus trunk and artery subclavia, caused by atherosclerosis (57), nonspecific aorto-arterition (36), and consequences of traumas of arteries (9). Investigation of regional and a brain haemodynamics has shown that occlusion of trunk brachiocephalicus and I – segment artery subclavia essential decrease a brain blood-groove for the account “steal syndrome” on vertebral and to sleepy arteries.
Choice of a method of operation depended on localization, quantities and extents of defeat of vessels. Shunting operative interventions were applied: intrathoracies (18) and extrathoracies (82). At defeats of left sleepy and subclavia arteries used link sided lateral thoracial access. At occlusion of distal portions of artery subclavia a choice method was roundabout autovenouse shunting.
Optimization of surgical tactics has allowed to reach good results in the nearest postoperative period at 93,1 %, in kept away – at 93,5 % of patients. -
The organization and activity of diagnostic service of the various countries (the literature review)
Thus, the literature review has shown that diagnostics as scientific discipline, developed in indissoluble communication with other scientific disciplines. The key moment of its development was the wide experience of great scientific Gippokrata, Galena, Avitsenny, and also the organization of school of medicine in the countries of the East, Asia, Europe. Formation of the diagnostic centers, as in Europe, America, and in the countries of the former USSR became an important stage of development of diagnostic service. Activity of the kliniko-diagnostic centers shows their conclusive advantage and rather considerable role in public health services system. Thus efficiency of activity providing at the expense of introduction of the expensive equipment, modern methods of research and presence of highly-skilled personnel.

SITUATIONAL ASSESSMENT OF THE RESULTS OF TREATMENT ABSENCE OF TEETH REMOVABLE DENTURES

To study the results of orthopedic treatment of occlusive defects removable dentures assessed the state of orthopedic status of the oral cavity in 198 patients aged 20 to 60 years and older. In patients examined dentures are the most used in combined defects of dentitions, singly standing tooth in the jaw, and at the end defects. The least dentures are used when included occlusive defects. The number of removable dentures made on the upper and lower jaw, respectively 49,3% and 50,7%. The share of clasp prostheses made of the lower jaw with partial absence of teeth, amounted to 54,6 per cent. In the absence of teeth laminar dentures in 61,4% of the cases were made in the upper jaw. The largest number of laminar dentures (37,4%) had a life span 3 to 5 years. There was a significant predominance of the laminar dentures in need of re-manufacturing, over clasp, the service life of up to 5 years. If a period of more than 5 years, the number of clasp dentures exceeds the number of laminar. Comparing the reasons for replacement of removable dentures with partial absence of teeth, it can be noted that the leading position clasp prostheses occupy complications associated with fractures of the abutment teeth, laminar – breakdown of prosthesis (38,7%) and aesthetic dis¬orders (13,6%). The main causes of alteration of a laminar dentures were bad fixation (45,4%) and breakage of prosthesis (29,4%). Aesthetic disorders amounted to 13,4%, 8,8% of cases the cause of re-prosthetics was a subjective desire of the patient, 2,9% of cases – other reasons. -

ULTRASOUND DIAGNOSTICS THYROID PATHOLOGY IN CHILDREN AND ADOLESCENTS

THE COURSE OF ENDOCRINOLOGY AT DEPARTMENT OF THERAPY TIPGPMP REPUBLICAN CLINICAL CENTRE OF ENDOCRINOLOGY
In connection with the exceptional importance of the thyroid gland problem of pathology of the organ of children is the focus of endocrinologists. Pathology of the thyroid gland in the regions of iodine deficiency, which includes the Republic of Tajikistan, occupies the leading position in the endocrine pathology in children and adolescents. Diagnostics of thyroid pathology in this category includes a phased investigation with the decision of the question concerning the goitrous increase the authority and assessment of the functional state of the thyroid gland. Many tasks successfully done with the use of ultrasonic technologies. The article suggests the use of ultrasound diagnostics for estimation of structural and functional changes of the thyroid gland, 93 children and adolescents with a duration of pathology from 6 months to 2 years. Depending on the functional state of the examined organ children and adolescents were divided into the following groups: I group amount¬ed to 61,3% of patients with diffuse nontoxic goiter, II – 20,4% of individuals autoimmune, with tireoidita III group – 18,2% of surveyed with hypopituitarism thyroid gland. Results of ultrasonic research show the greatest increase in volume of the thyroid gland in the group 2 patients with au¬toimmune thyroiditis. Analysis of results of ultrasonic research showed that in the 1 group of the examined enlargement of the thyroid gland was moderately expressed. The largest increase of the volume of the thyroid was noted in group 2 patients with autoimmune thyroiditis. Of the 17 patients with subclinical hypothyroidism last group in 58% of cases of thyroid volume slightly increased in 42% of cases, the value remains in the normal range. The results obtained allow to note that for the estimation of structural and functional changes of the thyroid gland are essential ultrasound diagnostics.
